Every day, the nurses and clinical staff at Florida Hospital Fish Memorial provide patients with critical information about ways to enrich their lives. Our patient education programs, screenings, events and seminars help us share this valuable knowledge with patients, guests and other members of the medical community.
 

Things to Know When You Get to the Hospital

Below, we offer some important information you’ll need when coming to our hospital. However, if you still have questions, please don’t hesitate contact us  directly. We’d be happy to discuss your concerns and will endeavor in all we do to make your Florida Hospital experience as comfortable as possible.

As our guest, we invite you to take advantage of our many Web services, such as e-cards for patients and online prayer requests. For our patients, we offer two unique sites to help you securely manage your patient health information, personal health records, pay bills and much more.

Patient Portal

In order to provide you with exceptional service and convenience, our Patient Portal  offers online scheduling and pre-registration for tests, management of personal health records and the ability to pay your hospital bill online from any personal computer in a safe and secure manner.

Below are examples of what our Patient Portal will enable you to do:

■Pay My Bill: Instant account access, where you can pay your account balances or view billing information for services.
■Request an Appointment: Need to schedule an appointment for one of our common procedures? Let us know when you’re available and we’ll get you right in.
■Personal Health Records: Are you managing your family’s medical information? Our online tool gives you a simple, safe and smart way to organize all the details.
■Pre-register for an Appointment: Save time by submitting your registration information in advance of your scheduled appointment.

MyAccess

We want to make accessing your health information easy—so you can get it when and where you need it. Through MyAccess, a secure and personalized website, you can view your health information online. If you are a patient and do not have a MyAccess account, you must request one to access these features (see instructions below).

MyAccess enables you to:

■Access your Medical Record Summary: Including allergies, discharge instructions, health issues, immunizations, medications and surgeries/procedures.
■Access Clinical Results: View lab results.
■Access Personal Information: Including personal data and insurance information.
■Whether you want to review your medications, discharge instructions or access your lab results, this convenient tool is available when and where you need it.
Requesting a MyAccess Account
For the security of our patients’ health information, online access through MyAccess requires patients to have an email address and must be requested using one of the following methods:

■During the admission process
■Via mail by printing and filling out a notarized authorization form
■Making an in-person request at the hospital’s Health Information Services Department
f you do not have a personal email account, you can always obtain your health information in person from the Health Information Services Department at the hospital.
For patients who are admitted to the hospital:

1.If you are admitted to the hospital or Emergency Department and are at least 18 years of age, you will be enrolled in MyAccess when you provide your email address to a registration team member.
2.We will then send an email to the address you provide with the personalized information you need to create a secure online account.
3.You will create your secure online account with a username and password.
4.You will receive a welcome email with a link that you can use to access your health information.
5.After that, you’ll be able to log in and securely access your personal health information whenever you need to.
For those requesting a MyAccess account by mailing in a notarized registration form:
If you do not have your personal email address available when you are admitted, you can enroll in MyAccess from home by following the simple steps below:

1.Download, print and complete the MyAccess registration form .
2.Get the MyAccess registration form notarized and mail it to 1055 Saxon Blvd., Orange City, FL 32763, care of Florida Hospital Fish Memorial's Health Information Services Department.
3.Health information services personnel will enter your information into MyAccess.
4.You will receive an email with the personalized information you need to create a secure online account.
5.You will create your secure online account with a username and password.
6.You will receive a welcome email with a link that you can use to access your health information.
7.After that, you’ll be able to log in and securely access your personal health information whenever you need to.
For patients who prefer to request a MyAccess account in person at the hospital’s Health Information Services:

1.Go to the Health Information Services Department and provide personnel there with your email address.
2.The health information management personnel will enter your information into MyAccess.
3.Using the email address you provide, we will send an email with the personalized information you need to create a secure online account.
4.You will create your secure online account with a username and password.
5.You will receive a welcome email with a link that you can use to access your health information.
6.After that, you’ll be able to log in and securely access your personal health information whenever you need to.

MedlinePlus directs you to information to help answer health questions. MedlinePlus brings together authoritative information from NLM, the National Institutes of Health (NIH), and other government agencies and health-related organizations. MedlinePlus also contains extensive information about drugs and supplements, an illustrated medical encyclopedia, interactive tutorials, the latest health news, and surgery videos.
